Updating ACP Information

When you move, you must notify the ACP at least two (2) days before you move. Address changes MUST be:

• Submitted in writing; AND
• Include your signature to verify your identity.

To make things easier, ACP offers a Change of Information Form on our website. Complete the form, print it, sign and submit the form one of the ways below. If you don't have a printer, use the form as a template of what information is needed. 

Your changes (form or template) can be:
