Updating ACP Information

When you move, you must notify the ACP at least three (3) days after you move. Address changes MUST be:

• Submitted in writing, on the Change of Information Form; AND
• Include your signature to verify your identity.

The Change of Information Form is on our website. Complete the form, print it, sign and submit the form one of the ways below.
